The information contained in this application is correct to the best of my knowledge. I hereby authorize A Safe Place and its designated agents and representatives to conduct a comprehensive review of my background causing a consumer report and/or an investigative consumer report to be generated for employment and/or volunteer purposes. I understand that the scope of the consumer report/investigative consumer report may include, but is not limited to the following areas: verification of social security number; current and previous residences; employment history, education background, character references; drug testing, civil and criminal history records from any criminal justice agency in any or all federal, state, county jurisdictions; driving records, birth records, and any other public records.
Non-solicitation/Confidentiality Agreement
I recognize that A Safe Place (ASP) is engaged in business that collects and reports confidential information, and that it is important for ASP to protect its confidential information and other proprietary information and related rights acquired through ASP?s expenditure of time, effort, and money. Therefore, because I seek to be an employee/volunteer/intern with The COR in a capacity where I will be exposed to, receive, and/or actively contribute to ASP?s confidential information, I agree to be bound by the following terms and conditions which are described below:

Definition of Confidential Information
In this agreement, ?confidential information? includes client and/or employee/volunteer/intern information, including (but not limited to) entering confidential information into a computer database.

Non-disclosure of Confidential Information
I agree to retain all confidential information in the strictest confidence. I will not disclose any confidential information to any person who is not an employee/volunteer/intern at ASP. In addition, any confidential information I disclose to an employee/volunteer/intern will only be done as needed when he/she requires said information to perform his/her duties to ASP. Further, I understand that any breach of this confidentiality agreement could result in my immediate release or termination, as well as legal action against me.
I have read this agreement and agree to abide by its terms.
